NR enters circulation the moment the infusion begins. In the 2024 pilot trial, whole blood NAD+ peaked more than 20% above baseline around the three-hour mark. Oral capsules have to clear stomach acid and first-pass liver metabolism before any NR reaches the bloodstream, so peak timing here is measured in hours rather than days. What that biomarker change means for how you feel was not an endpoint of the study.

NAD+ is a large molecule that is largely broken down outside the cell before it can be taken up. NR enters cells intact and is converted to NAD+ internally. In the pilot trial, the NR arm showed a higher whole blood NAD+ response, fewer and less severe infusion-related experiences, and a mean tolerable infusion time 75% shorter than the NAD+ arm.

NR does not activate the GPR109A pathway responsible for the flushing associated with nicotinic acid, which is what separates it from niacin.
